The 12-month membership plan fee is due in full upon joining. This plan can be combined with dental insurance. Membership is effective immediately when payment is received. Payments are due when additional services are rendered, and it is the sole responsibility of the member to maximize their benefits by arranging all the necessary appointments within the 12-month membership period. If the appointments are not used, the member will not be entitled to a refund. The 10% treatment discount may not be combined with third party payment plans. Renewal payment is due the beginning of the same month each year.
